Contact form

An enquiry form — full name, email, phone and a message box, with a submit button. Someone fills it in, and the message arrives by email at the address set under Settings → Store details → Sender email in the Shopify admin.

This is not a newsletter signup. It doesn’t add anyone to a marketing list and it doesn’t create a customer record — it sends a one-off message. The newsletter signup in the footer is a separate block that goes to Klaviyo.

Setting What it does
Show name field On by default. Turn off to drop the name field entirely.
Name label The label above the name field. Default “Full name”.
Make name required Off by default. When on, the form won’t submit without a name and the label gets an asterisk.
Email label The label above the email field. Default “Email”.
Show phone field On by default. Turn off to drop the phone field.
Phone label The label above the phone field. Default “Phone”.
Make phone required Off by default.
Message label The label above the message box. Default “Comments”.
Make message required Off by default.
Setting What it does
Button label The text on the submit button. Default “Submit”.
Button alignment Left (default), Center or Right.
Setting What it does
Success message Shown in place of nothing above the form after a message sends. Default “Thanks — we have your message and will be in touch shortly.”
Disclaimer Optional small print below the fields — a privacy notice, say. Accepts rich text and links.

The field list is fixed — name, email, phone, message. There’s no way to add a dropdown, a checkbox, or a second text field.

If you need custom fields, conditional logic, or submissions stored somewhere you can export them, use the Shopify Forms app block instead. A Column accepts app blocks, so it drops into the same place this one does.

  • Say what happens next in the success message. “We’ll be in touch” is better than “Thanks”, and “within one business day” is better still — it stops people submitting the form twice.
  • Turn off fields you won’t act on. Every field is a reason to abandon the form. If nobody calls people back, drop the phone field.
  • Use the disclaimer for the privacy link. Anywhere collecting an email is a reasonable place to link the privacy policy.
